This comprehensive pharmacist resource created by Teva Canada gives you everything you need to implement a successful smoking cessation program that meets ODB requirements. Each tool kit contains a backgrounder (with clear instructions on how to implement, document and bill the program), a checklist for patient interactions, an appointment sheet and appointment cards.
